8/8/22, the CREDIT CARD ON FILE POLICY will be enforced. New Patient NO SHOWS or CANCELLATIONS within 24 hours result in a $50 fee.
ESTABLISHED PATIENTS FROM OLD PRACTICE: Fill out forms, fax authorization to (941) 342 9488. Request 1st note, last note and all tests.
Personal injury, medicolegal, and workers compensation cases are not seen.
Office hours: 9am-1pm. Save non-urgent questions for next visit. Call for administrative issues 9-1pm M-F. Non-urgent clinical issues are discussed during appointments. Patients requiring routine clinical issues addressed quickly are better suited to a large practice. A call initiated by the patient resulting in the doctor’s provision of medical advice or discussion which requires medical documentation will be charged. Appointments are moved up for urgent test results. Administrative issues are addressed in between appointments without fees.
Calls are taken for scheduling and urgent matters 1-5pm.
Concerns:
- KINDNESS MATTERS: Verbal abuse, rudeness or demeaning of staff is not tolerated and leads to discharge.
- INSURANCE ISSUES and CHARGES: The office checks deductibles, copays, referrals and prior authorizations. You are responsible for fees not covered by insurance.
>5 minute phone calls resulting in a medical note are charged.
>30 minute non-face-to-face work (records review, speaking to doctors, prior authorizations etc.) incurs a fee.
Patients must know their own insurance. We pre-check insurances, but the final answer is after claims are submitted.

Patients are given reminder appointment cards. Courtesy reminder calls are made but it is your responsibility to know your appointments. Also, you can place date immediately in your phone or place reminder card in your wallet.
